If you are an established patient, we ask that you contact us through your secure patient portal, where you can connect with your provider, nurse and/or appointments. This “contact us” feature is for requests for new-patient inquiries, questions about our practice or providers, and/or policy inquiries.
The email link is not encrypted/secure. All information with personal health information (PHI), or personal identifying information, should not be included in your email or in attached documents. Examples of PHI or personal identifying information are dates of birth, personal medical information, or even insurance information. This information should not be sent in an email, but transmitted only through an encrypted email, brought into the office or sent US mail. Website inquiries are not routinely checked. If you have any situation that needs immediate attention, please call the office.
