Navigating Back-to-School, Sports, and Other Forms

Ah, paperwork. Just the word can sometimes bring a sigh! It’s true that as the school year wraps up and the allure of summer fun takes center stage here in North Carolina, those essential sports physicals and back-to-school forms can easily slip down the to-do list. Before you know it, the calendar flips, and a last-minute scramble ensues to get everything completed. 

We hear you, and we want to make this process as smooth as possible. So, we’ve compiled some of the most frequently asked questions we receive from our wonderful parents and caregivers, on, you guessed it, forms!

When should I schedule my child’s back-to-school physical?

Now is the perfect time! Our appointment slots fill up quickly as we get closer to the start of the school year. Planning ahead and scheduling your child’s physical early ensures they are ready to go and helps you avoid any last-minute rushes. You’ll be cool as a cucumber when August rolls around and everyone else is stressing to get any necessary paperwork completed!

What do I need to bring to my child’s school or sports physical appointment?

To make the most of your appointment, please bring:
•Any school or sports forms that need to be completed.
•Your child’s most up-to-date immunization records, if you have them, and you are a new patient of our practice.
•A list of any current medications your child is taking, including dosages.
•Any specific concerns or questions you have for the pediatric provider who is examining them.

How long does it take for Rainbow Pediatrics to complete school or sports forms?

Please allow 3-5 business days for us to complete school and sports forms. Our team works diligently to get these done accurately and efficiently while also providing care to all our patients.

What about FMLA or EFMP paperwork? How long does that take?

Forms such as FMLA (Family and Medical Leave Act) and EFMP (Exceptional Family Member Program) often require more detailed review and documentation. Please anticipate a longer processing time of 10-14 business days for these types of forms.

Can I drop off the forms and pick them up later?

Yes, you can certainly drop off the forms at our front desk during our regular business hours. This helps us manage the workflow efficiently. Please ensure that all necessary sections requiring your input are completed before you drop them off. We will notify you when they are ready for pick-up.

My child needs a physical examination as soon as possible for a summer sports camp. Can you accommodate a same-day request for the forms?

While we always strive to accommodate our patients’ needs, completing forms requires careful attention to detail and review by our providers. Same-day completion can be challenging due to the high volume of requests and our commitment to thoroughness. We strongly encourage you to submit forms as early as possible to avoid any delays.
