We’re excited to welcome your family to Rainbow Pediatrics. To get started, all new patients must complete our New Patient Forms before scheduling an appointment.

Download the New Patient Forms using the link below, complete them in full, and email the forms to newpatient@rainbowpeds.net
Once submitted, our team will review your information and be in touch within 48 hours to help schedule your child’s appointment.
Call the Clinic
Prefer to speak with someone? Give us a call and our team will guide you through the process and direct you to complete the required forms.
Need a Sick Visit?
If your child is sick and needs to be seen, please call the clinic directly.
Our team will help coordinate care as quickly as possible.

Deductible & Co-Payments
All deductibles and co-payments are due at the time services are provided.
If you don’t have your insurance card at your visit, you’ll need to provide enough information for us to verify coverage and file claims. Otherwise, payment in full will be expected at the time of service.

Our Vaccine Policy
Vaccines are one of the safest and most effective ways to protect your child’s health. At Rainbow Pediatrics, we follow the AAP recommended immunization schedule to keep our patients, families, and community safe. Because we believe strongly in this standard of care, we require that all patients stay up to date on their vaccinations.
If you choose not to vaccinate, we understand that every family has different beliefs, and we may not be the right fit. In those cases, we kindly ask families to find a healthcare provider whose approach matches their own.
