    Call the office location where you would like to establish treatment.  They will mail you a New Patient Questionnaire.  When we receive the completed questionnaire, it will be review for approval.  A member of our staff will contact you to schedule an appointment.  This process usually takes about a week.  Occasionally, we do limit the number of new patients depending on the location and circumstances.
